Hemşirelikte Etik ve Değerler Dersinin Hemşirelik Öğrencilerinin Etik Karar Verme Süreci Üzerine Etkisi

Hemşirelikte etik eğitimi ile yüksek kalitede bakım sunarken etik açıdan duyarlı profesyonel hemşirelerin yetiştirilmesi amaçlanmaktadır. Bu nedenle araştırma vaka analizleri ile yürütülen hemşirelikte etik ve değerler dersinin öğrencilerin etik karar verme süreci etkisini incelemek amacı ile planlanmıştır.Araştırmanın evrenini bir Sağlık Bilimleri Fakültesi Hemşirelik Bölümü’nde eğitim gören ve etik dersini ilk defa alan 87 2. sınıf öğrencisi oluşturmuştur. Araştırma için örneklem seçimine gidilmemiş ve evrenin tamamına ulaşılması hedeflenmiştir. Ön-testte toplam 80 (%92 katılım) öğrenciye ulaşılmıştır. Son-testte ön teste katılmış öğrencilerin 69 (%79 katılım) tanesine ulaşılabilmiştir. Verilerin toplanmasında “Kişisel Bilgi Formu” ve “Hemşirelikte Etik İkilem Testi” kullanılmıştır. Araştırma kapsamına alınan öğrencilere dersin ilk haftası başlamadan önce “Yapılandırılmış Kişisel Bilgi Formu” ve “Hemşirelikte Etik İkilem Testi” uygulanmıştır. Ders araştırmacılar dışında bir öğretim elemanı tarafından yürütülmekte ve kapsamında teorik bilginin ardından çok sayıda sık karşılaşılan etik vakaların analizleri ile konular işlenmektedir. Derslerin tamamlanmasını takiben dönemin son haftasında aynı öğrencilere tekrar “Yapılandırılmış Kişisel Bilgiler Formu” ve “Hemşirelikte Etik İkilem Testi” uygulanmıştır. Etik ikilemler karşısında ne yapmalı konusunda sadece ilaç hatalarını bildirme konusunda anlamlı düşüş tespit edilmiştir. Öğrencilerin ilkesel düşünme puanlarının ortalamasının etik dersi öncesi 49,26±6,08; etik dersi sonrası ise 54,79±8,91 olduğu ve öğrencilerin puan artışlarının istatistiksel olarak anlamlı artış gösterdiği belirlenmiştir (p:0,001). Pratik düşünme ve aşinalık puanları artmasına rağmen istatistiki olarak anlamlı bulunmamıştır (p>0.05). Sonuç olarak, etik ikilemler karşısında ne yapmalı konusunda sadece ilaç hatalarını bildirme konusunda anlamlı düşüş tespit edilmiştir. Öğrencilerin etik dersi öncesi ve sonrası ilkesel düşünme puanlarının ortalamanın üstünde olduğu ve etik dersi sonrasında anlamlı olarak artış gösterdiği belirlenmiştir.

Through education on ethics in nursing, raising professional nurses who are ethically responsive while providing high quality care is targeted. Therefore, the research was devised to analyze the effect of the course of ethics and values in nursing on the development of the ethical decision-making ability of students. Research population was composed of a total of 87 two-year students who took the course of ethics for the first time and were enrolled at the Department of Nursing of a Faculty of Health Sciences. A sample was not selected from the population in the research, rather the research was designed to study the entire population. 80 students, namely, 92% of the entire population, participated in the pre-test phase of the research whereas 69 of those students who participated in the pre-test phase, namely, 79% of the entire population, joined the post-test phase. In order to collect data, ‘Personal Information Form’ and ‘Ethical Dilemma Test in Nursing’ were utilized. Prior to the launch of the first week of the course, students participating in the research were asked to fill in the ‘Structured Personal Information Form’ and to take ‘Ethical Dilemma Test in Nursing’. In the last week of the semester after the completion of sessions of the course, the same students were required once again to fill in the ‘Structured Personal Information Form’ and to take ‘Ethical Dilemma Test in Nursing’. It was discerned that the mean of students’ scores of ‘principled thinking’ was 49.26±6.08 before the course and 54.79±8.91 after the course, and there was a statistically significant increase in students’ scores of ‘principled thinking’ (p: 0.001). In conclusion, as to what to do when confronted with ethical dilemmas, it was found that there was a statistically significant decrease only in reporting medication mistakes. It was ascertained that students’ scores of principled thinking were above average both before and after the course and there was a statistically significant increase in students’ scores of ‘principled thinking’ after the ethics course.
